If you won the Mega Millions lottery, what's your first act?

In the hopes of striking it rich, people across the country are scooping up Mega Millions tickets. NBC's Kevin Tibbles reports.

By Elizabeth Chuck, msnbc.com

Venus Wilson has been fantasizing about winning the Mega Millions lottery. The first thing she would do? "Faint!" the security officer said at her post outside of JPMorganChase in Midtown Manhattan on Friday. "Then, when I get up, go get my check, and go shopping."

Wilson, 31,??has bought seven lottery?tickets. Other than passing out, she's got big plans if one ends up being a winner: She'd invest in real estate, but first, she would make sure her friends and family were taken care of.

"I would donate to St. Jude's for cancer research, for breast cancer, do stuff like that. Because come on, why not? Don't be stingy. God didn't bless you with it for no reason. Help people out. And I would pay off my parents' doctor bills, and I would help family and friends," she said.?"Then I?would go crazy [shopping]. I swear --?I would go crazy."

She also has plans to keep the cash flowing: Wilson wants to open soul food restaurants and laundromats, and get an accountant to invest her leftover winnings.

"You have to keep it coming. A lot of people, they win all that money, and then they're broke," she said. "Some people don't believe in the lotto. But look at the gold pot. It's like the leprechaun with the rainbow. That pot of gold is beautiful right now," Wilson said.

Wilson was among the New Yorkers canvassed Friday about?the?first thing they would do if they won Mega Millions, which has?hit $640 million. ?Hundreds more gave their responses on our Facebook page.

What to buy when you win the jackpot

'A dollar and a dream'
Therese Schoenwandt, 29, has been swept up by the lottery fever. The nursing student got engaged two weeks ago and sat in Barnes & Noble on Friday, flipping through "Wedding Planning For Dummies."?

Elizabeth Chuck / msnbc.com

Therese Schoenwandt, 29, a nursing student who recently got engaged, purchased a lottery ticket for the first time this week.

"I've never done a lotto ticket before. I usually don't believe in that stuff," she said. "It's all over the papers, and it's all over the news. Yesterday when I saw people lining up outside stores for it I said, 'You know what, I'm going to buy a ticket.' Peer pressure!"

Her first act if she wins: Pay off her mother's mortgage in Brooklyn. The she will plan?her wedding, and put the rest in the bank.

Ivan Martinez, 50, who was handing out fliers in Manhattan's diamond district, had high hopes of helping out his family back home in Puerto Rico with the one ticket he purchased earlier in the day. "It's a dollar and a dream," he said.

Richard Gallo, a 47-year-old customer service representative in the watch industry, said?if he won, the first thing he would do is jump in the car and drive. "Before I even turned the ticket in, I would take a trip. Wherever I ended up, I ended up. Basically just to step away from everything, think about it and come back. You get caught in that whirlwind."

Others weren't as confident about the dream's possibility. "It's all fixed," said a man who refused to give his name but was filling out his numbers for a ticket. "Do you know anyone who's won? It's all fixed, if you ask me." Then he continued filling out?his ticket.

Personal finance expert Suze Orman weighs in on the Mega Millions craze.

On our Facebook page, many readers weighed in: "Sign the back of the ticket. Then go on a vacation to clear my mind while deciding who a good financial adviser would be to go to," wrote?Christopher Ringen.?

Others said they would pay off debt for family and friends or their churches, or donate to animal shelters. For some, though, it was just about making life a little more comfortable: "Never fly coach again," wrote Rebecca Hayes.

But not everybody has bought into the hype. "It doesn't really attract me," said Louis Carrasquillo, 50, a sales rep at Montecristo Cigar Shop on 5th Avenue in New York.

"Have I thought about it? Probably. I passed by the booth and I thought it would be great, but I'm just a working person. I'm not really a gambler."

If he did win that kind of cash, he would give money to his church for a new school and cafeteria, he said.

As the Mega Millions Jackpot lottery winnings continue to grow, the NOW panelists discuss how the lucky winner is not the only beneficiary of the lucrative system.

But?Carrasquillo?said he was turned off?by?some lottery tips he heard from a?newscaster: "If you're going to get divorced, get divorced before you win. What's that about??Change your phone number, 'cause you're going to hear from family members you haven't heard from. You gotta be kidding me."

Why even skeptics are rushing to buy a Mega Millions ticket

Mega Millions is played in 42 states plus Washington, D.C. Friday's drawing happens at 11 p.m. ET. The largest Mega Millions jackpot ever won was $390 million in March 2007, when the prize was split between two tickets sold in Georgia and New Jersey.

Teens rule in the bloody 'Game of Thrones'

By Susan C. Young, TODAY.com contributor

HBO

Jack Gleeson as Joffrey Baratheon, Emilia Clarke as Daenerys Targaryen and Maisie Williams as Arya Stark on "Game of Thrones."

Kids grow up fast in the world of Westeros on HBO's "Game of Thrones."

One day, their worst fear is disappointing Dad, and the next they watch as he gets a public beheading and his noggin ends up mounted on a spike where the whole community can see it.

Barely a teen, Sansa Stark (Sophie Turner, 16) tries to remain calm when her 13-year-old betrothed, the not-quite-stable King Joffrey Baratheon (Jack Gleeson), degrades her and forces her to live in fear after ordering the gory demise of his future father-in-law, Ned Stark. Meanwhile, her little sister, Arya, 11 (Maisie Williams, 14), boldly escapes with a ragged clan of miscreants to find what remains of her family.

It?s a lot for kids to take on.

While no one would call ?Game of Thrones? a teen drama, there?s no denying that the majority of characters are in the under-19 demo (many ranging from children to barely teens), and they carry central roles.

After the death of their daddies last season, the Stark and Baratheon kids are anything but all right when the story picks up on Sunday. ?

The season begins as teenager Robb Stark has taken up the banner as the King in the North, while his similarly aged bastard brother Jon Snow soldiers on as a member of the Night's Watch, which holds the line between the lands to the south and the otherworldly dangers to the far north with a great wall. Fellow teen Theon, a noble hostage raised as their sibling, has been sent back to his home to gather the troops in support of faux-bro Robb.

And off in the hinterlands, teen widow Daenerys Targaryen, who is just 15 (played by Emilia Clarke), is leading a ragtag group of Dothraki followers, with her fledgling trio of dragons in tow.

These are just some of the younger players featured on "Game of Thrones" this season. There are more school-aged characters on the show than you?ll find on an average CW series, with even more sexual intrigues and teen angst. And, as often happens, most of those older teens are played by young adult actors in their 20s.

As impressive as the adult cast is, it?s the child actors who really stand out as they play complicated characters with an ease that belies their age. The complex series has dozens of parts, and these kids play a major role in advancing the story.

In TV, you get used to the young actors mugging it up for the camera, not exactly showing a range of emotions that would lead to an award-winning performance. The rare recent exceptions include Kaitlyn Dever, who played orphan Loretta in ?Justified? when she was just 14 and stood toe-to-toe with Emmy winner Margo Martindale, and ?Mad Men? phenom Kiernan Shipka, 12, who has been wowing critics for a few years as Don and Betty Draper?s daughter, Sally.

In ?Game of Thrones,? the producers scored with a young cast that hits it home every week. Williams? portrayal of one of the most popular characters in the book, Arya, doesn?t miss a step as the feisty little girl disguises herself as a boy as she fends for herself.? It?s a meaty role that could go so terribly wrong in the hands of a lesser actor.

The same holds true for Turner as Sansa, the beautiful spoiled daughter of Ned Stark who dreams of marrying a prince and being a good wife, mother and princess.? She shows her mettle when she?s forced to play her own game of survival within the walls of her enemies.

Little brother Bran (Isaac Hempstead-Wright, 12), who would probably be a fourth-grader in modern times, is left at home to rule Winterfell, the seat of House Stark. He?s soldiering on despite being a cripple, the result of getting tossed out of a window after seeing the fornication between twins Jaime Lannister and Queen Cersei Baratheon, Joffrey?s true parents.

Now that?s something that can leave a mark.

Gleeson, who is in his late teens, shines as the sadistic Joffrey. In a TV season filled with hateful male teens from ?Smash,? ?The Killing? and ?Terra Nova,? Gleeson?s portrait rings true without the feeling that he?s just there to fulfill some network demographic need.

And as the war rages on in season two of "GoT," it looks like the game will be well played by the youngest members of this compelling drama.

Chilean astronomer makes her mark

Chilean researcher Cinthya Herrera has not quite achieved her PhD in astronomy yet, but already she has notched up a notable success in her career.

The 27-year-old student has the distinction of publishing the very first science paper to come out of the Alma radio telescope.

This huge facility, going up in Chile's Atacama Desert, is set to revolutionise our understanding of the cosmos.

Ms Herrera's efforts will be followed by thousands more publications.

"I was really excited to be told my work was the first refereed paper accepted for publication based on Alma observations, but also I was extremely proud because Alma is in Chile," the young astronomer told BBC News.

Ms Herrera has been describing her studies here in Manchester at the UK National Astronomy Meeting (NAM).

Her investigations involved looking for star forming clusters resulting from the merger of a pair of spiral galaxies.

This collision, known as "The Antennae", lies about 70 million light-years away in the constellation of Corvus (The Crow).

It produces very turbulent regions of gas that must dissipate their energy if they are to condense and form new stars.

"With Alma and its wonderful resolution, we were able to trace the molecular mass of the gas and the structures that will form stars; and using another telescope run by the European Southern Observatory, we were able to trace the energy dissipation," Ms Herrera explained.

"The tracers we observe to do this are carbon monoxide in the case of Alma and molecular hydrogen in the case of the second telescope - the Very Large Telescope, also in the Atacama."

Details of the work have appeared in Astronomy and Astrophysics.

The data was acquired in the so-called science verification phase of Alma (Atacama Large Millimeter/submillimeter Array).

The co-operative venture that includes the scientific and engineering inputs of Europe, East Asia, North America, and the host nation, Chile, is still only half-built.

Week by week, new radio antennas are being added to the observing network sited 5,000m above sea level on Atacama's Chajnantor plateau.

Right now, there are 22 of its 12m dishes in place with another seven 7m dishes also observing the sky.

It is planned there should be a total of 66 antennas when Alma becomes fully operational in the next couple of years.

The observatory's capabilities are keenly awaited by astronomers.

The unprecedented resolution it will achieve at longer wavelengths of light will allow scientists to study extremely cold objects in space - such as the dense clouds of cosmic dust and gas from which stars and planets form.

Pallab Ghosh looks around the array of giant antennas that makes up Alma

It is expected also to see very distant objects in the early Universe, including some of the very first structures to form more than 13 billion years ago.

Ms Herrera is currently studying in Paris, France, at the Institute of Space Astrophysics (IAS), but her intention is to return home to work on all the telescopes that take advantage of the great observing conditions in the high, dry Atacama.

"For the next few decades, I think Alma will be one of the greatest telescopes on Earth," she told the BBC.

Action For Better Healthcare ? Premier and CaroMont Health named ...

Tuesday, March 27th, 2012

Friendly competitions that compare steps taken, employee sports leagues, on-site exercise classes and discounted access to fitness classes ? these are just some of the offerings from companies that are touted as the healthiest employers in Charlotte, NC.

The Premier healthcare alliance was named Charlotte?s Healthiest Employer among large businesses by the Charlotte Business Journal. Alliance member CaroMont Health also ranks a close second on the list. The companies were recognized for their best practices in providing wellness initiatives in the workplace.

Premier?s 1,300 employees take steps to improve their health by challenging themselves each day to reach specific fitness goals. Among other efforts, employees wear wireless pedometers that track how many steps they take. Each employee has a goal of taking at least 2 million steps a year, with an overall company goal of at least 2.6 billion steps by the end of 2012.

?If employees meet their individual annual goals, and improve or maintain their biometric health status, they receive $500.

?Studies show our healthcare system wastes as much as $1.2 trillion annually. Obesity and related issues are a big part of this waste,? said Premier President and CEO Susan DeVore. ?While Premier?s mission is to help hospitals and healthcare systems improve health across America, our own employees have challenged themselves to lead as examples by practicing what they preach. Our efforts are part of a national imperative to bend the healthcare cost curve and improve the nation?s health status, starting within our own community. We?re proud that our hard work is being recognized.?

To help employees eat a healthier diet, in addition to exercising more, Premier also offers healthier vending machine and dining options. Employees enjoy free fruit every Monday, and buy lunches from local restaurants that bring healthy dining options on-site.

Premier?s focus on wellness is paying off. The cost of employee insurance claims has remained flat or increased at a rate below the industry norm of 8 percent a year. And the percentage of employees at risk for developing a health condition has decreased in all but one category, according to employees? biometric screening trends.

Amateur Radio = Communications Resilience ? Sierra Permaculture ...

Amateur Radio for communications resilienceOver the dozen or so years that I have been studying and applying sustainable design principles, I have observed that one of the permaculture community?s weakest areas of focus is in the realm of developing resilient communications systems. Here in the relatively affluent western world, we take it for granted that telephone landline, cell phone and internet service is available 24/7/365, all of which is possible due to the readily accessible and affordable energy that is currently at hand. But what is the likelihood that these convenient communications channels will be available in a world of increasing energy access constraints? Will the server farms and cell towers continue to function? Besides the energy issue and fear of a potential ?Zombie Apocalypse?, how can we stay in touch with our friends and family while also keeping important information flowing when natural disasters such as earthquakes, wildfires and other catastrophies occur at the local and regional level?

Building redundancy into our systems is a key part of permaculture design. When one or two elements fail, fall-back strategies help keep the overall system functioning. This I believe, is why amateur radio is so important and should be adopted as an integral part of everyones? overall design system, both at the home and community level.

I personally caught the radio bug at a young age. My father, who was an immigrant from Eastern Europe, brought his console radio with him when he moved to the United States. Included in the cabinet was not only a turntable and standard AM/FM broadcast receiver, but also a shortwave receiver with a graphical display of all the frequencies that could be tuned to in order to receive broadcasts from around the world. I was fascinated by the notion that it was possible to hear stations from Mexico, Canada and Europe and I?d listen to them whenever the ionospheric conditions allowed these long-distance transmissions to be heard. In all likelihood, it was this early exposure to radio that motivated me to become a radio broadcaster at KZSC 88.1 FM during my college days. After college, I purchased a shortwave radio of my own and challenged myself to hear DX (long-distance) broadcasts from around the world, I got to guest DJ for a neighbor at Burning Man who was operating a 1-watt FM ?pirate? radio station, and later, I toyed around with CB radios, only to find the 40 channels there polluted with cussing loud-mouths using modified rigs and 100 watt linear amplifiers which prevented anyone else from getting a word in edgewise. It was impossible to cut through all the noise and have an intelligent conversation. Then I discovered the amateur radio service!

At first, the notion that I had to take a technically-oriented test in order to get my entry-level ham ?ticket? was off-putting, but I eventually put my mental block aside after I realized that hams take the art and science of radio much more seriously than do the jabber jaws of the CB band, and not only that, knowledge of morse code was no longer required! If passing a simple 35 question multiple choice test (the FCC makes the question pool and all the answers available to you in advance) to get a Technician?s license was the only thing getting between me and a free, decentralized, wireless communications system, I eventually had to ask myself, ?What the heck am I waiting for??.

So I ordered a copy of the ARRL?s Ham Radio License Manual and began devouring the info. The great thing about this book is that as you read about licensing regulations, radio theory, safety and good operating practices, you are referred to the relevant test questions that appear in the appendix at the back of the book. To my relief, I discovered that there really isn?t all that much math or knowledge of electronics involved. It took me about one month of on-and-off studying to get through, and at the end, I loaded the book?s software CD onto my laptop so that I could take randomized practice exams. By the 5th or 6th try, I was aceing them and fully confident that I would do just fine on the actual test day. I contacted my local club, the Nevada County Amateur Radio Club, to let them know I planned to attend their next test session and showed up on the day of the test with money, pencil, and photo ID in hand. The club?s Volunteer Examining Committee (VEC) charged me just $15 to protor the test, grade it and submit my test results and license application to the FCC. The license itself doesn?t cost a penny and it lasts for 10 years, at which time you must renew it through the FCC for another decade. The room was quite full, with both aspiring hams going for the entry-level Technician class license, and those who were testing to upgrade to either a General or Amateur Extra License Class which would give them additional operating privileges on the HF (high-frequency) bands. My studying paid off! I scored 35/35 on the Technician exam and was given the option to test for the General license, which I took them up on at no extra charge. Without even studying, I scored 20/35 ? not quite enough to qualify, but not too shabby considering that I hadn?t even read up on the General class info. Once all my paperwork was submitted by the VEC, I waited 6 days and my new call sign was posted on the FCC?s amateur radio license database: KJ6UPR. Now I could finally start operating!

So what are you waiting for? Cultivating communications resilience is easy and it doesn?t take a big investment in gear. In my next entry, I?ll detail my particular choice of gear, where I got it, and how I plan to use it for communications beyond voice: file sharing, text messaging, email and even internet access!

73,

Paul

Pope Benedict visits Mexico

Updated at 5:12 pm today

Pope Benedict has arrived in Mexico on his first visit to Spanish-speaking Latin America, where the Catholic church is struggling to retain its influence.

Pope Benedict XVI arrives at Miraflores College in Leon, Guanajuato state, Mexico on his first trip to Spanish-speaking Latin America which will also include a two-day visit to Cuba.

PHOTO: AFP

He was welcomed by cheering crowds at the airport in Guanajuato state, the BBC reports.

President Felipe Calderon says the Pope's visit has enormous importance as Mexico struggles against drug-related violence.

The Pope has denounced drug culture, saying it is destroying Mexico's younger generation.

He also says Cuba, where he heads to after Mexico, has to turn its back on Marxism, which he said no longer corresponded to reality.

Ten years ago: President George W. Bush, during a visit to South America, pledged cooperation with Peru in the fight against terrorism. Girls in Afghanistan celebrated their return to school for the first time in years. Irina Slutskaya captured her first world title, defeating four-time champion Michelle Kwan at the World Figure Skating Championships in Nagano, Japan. Opera and pop singer Eileen Farrell died in Park Ridge, N.J. at age 82.

Five years ago: The House voted for the first time to clamp a cutoff deadline on the Iraq war, agreeing by a thin margin to pull combat troops out by late 2008. Iranian forces captured 15 British sailors and marines who were searching a merchant ship in the disputed Shatt Al-Arab waterway Persian Gulf; they were held for 13 days. Miss Tennessee Rachel Smith was crowned Miss USA at the pageant in Los Angeles.

One year ago: NATO ships began patrolling off Libya's coast as airstrikes, missiles and energized rebels forced Moammar Gadhafi's tanks to roll back from two key western cities. A blast blamed on Palestinian militants ripped through a bus stop in Jerusalem, killing Mary Jean Gardner, a 59-year-old British tourist and wounding two dozen other people, including five Americans. Army Spc. Jeremy Morlock pleaded guilty at his court-martial at Joint Base Lewis-McChord in Washington state to the murders of three unarmed Afghan civilians (he was sentenced to 24 years in prison).

SAN DIEGO -- The decisions counties are making right now about how to implement realignment will have dramatic and long-lasting impacts on public safety and on local taxpayers. In a new 100-page report, Public Safety Realignment: California at a Crossroads, the ACLU of California finds that many counties have yet to embrace a central charge of the realignment legislation: to implement evidence-based practices, including alternatives to incarceration, that are proven to prevent crime, limit future victims and stem the flow of people who cycle through our jails and prisons.

The new report, which is based on an in-depth review of all 53 available county realignment implementation plans, finds massive new investment in jails with much less funding allocated to proven crime-prevention programs such as mental health services and drug treatment.

?Simply building new jails or re-opening unused jail space treats the symptom but not the underlying disease,? said Margaret Dooley-Sammuli, Senior Policy Advocate of the Criminal Justice and Drug Policy Project of the ACLU of California and co-author of the report. ?It?s time to confront the fact that in California, over-incarceration is itself a disease, and the way to end it is to expand the use of mental health services, drug treatment and job training, and to reserve prison and jail for responding to serious crimes.?

Central to the ACLU?s analysis is the stunning fact that most people in county jails have not been convicted of a crime. More than 71 percent of the 71,000 Californians held in county jails on any given day are awaiting their day in court. Most of them do not pose a risk to public safety but are stuck behind bars because they simply cannot afford bail.

And while much of the conversation about realignment has focused on jail overcrowding, the ACLU argues that counties can significantly lessen the problem by adopting a pre-trial assessment tool to identify individuals who can be safely released on their own recognizance.

The report also finds:

? a promising commitment?though not yet realized?by many counties to adopt alternatives to incarceration and evidence-based practices to reduce recidivism;
? a troubling lack of state monitoring, data collection, outcome measurements and funding incentives to help counties successfully implement realignment; and
? paradoxically, under the current funding formula, a financial reward for counties that have historically incarcerated more people for low-level offenses.

California at a Crossroads offers recommendations for making the criminal justice system more effective ? and more cost effective.

The report also presents county-by-county profiles of the spending priorities established in the 25 counties that have initially received the most AB 109 dollars?nearly $400 million, slated to grow to more than $1 billion annually in the next fiscal year.

The 25 county profiles also contain a summary of jail expansion plans and notable efforts to expand the use of alternatives to incarceration.

Home Improvement Roofing Tips You Can't Do Without! | Pittsburgh ...

If you?ve ever heard the old adage ?measure twice and cut once,? then you?re well on your way to learning the correct way of making home improvements. That isn?t all there is to know, however. So find out what you don?t know in the article below.

Roof Ideas

If you have chosen to replace your roof shingle tiles with a metal one and need a vent there are different types that you can use. For each type of roof type you will need to choose the one that will be the most efficient for you. Do your research and choose wisely. When it comes time to re-do your roof, put up white tile, or another light colored substance. The lighter color will reflect the suns rays instead of absorbing them, reducing the heat build up in your attic. You can save a substantial amount on your monthly heating and cooling bill.

If you?re doing home improvements to help the sale of a home, you must focus on aesthetics, but you should also have a home inspection, just in case something serious needs to be done, as well. If you can install a new roof or furnace, it will show the buyers that you were on top of your game and will permit you to command a higher price.

Hose down your home on the outside using a pressure washer and detergents made specifically for your siding. Pressure washers can be rented quite economically and they do wonders in cleaning the girt off the outside of your home. Be careful around the roof line and windows. You don?t want to knock anything loose!

When you are planning a roof replacement, make sure to consider using white tiles. Not only does this give your home a unique look, it also translates into keeping your home cooler. Roof tiles that are a lighter color attract less heat, which reduces the heat that builds up in your attic or upstairs floor. With white tiles, your house is sure to stay cooler during the hotter months of the year.

Roof improvements can be an excellent choice for a home improvement project. Consider choosing white tile to re-do your roof. Tiles that are lighter in color can reduce the amount of heat that is pulled into your attic. This helps to save you money on your energy bills in the summer.

Always take your time. Unless it?s an emergency, like a leaky roof or flooded basement, plan ahead of a project at least three months out. Invest more time in order to save money by shopping around. Research what needs to be done and what materials are recommended so you have a plan and a shopping list once you are ready to begin.

Do you want to gain a little closet or floor space in your next home improvement project? Relocate your water heater to your attic! Modern water heaters are produced in efficient, reliable ?low-boy? designs that you can tuck safely between your ceiling and your roof. This allows you to gain a little extra storage space.

If you have a leaky roof, some of the first places you should look for the culprit would be skylights, around the chimney, wall step flashings, and low spots. The next place to look would be the gutters! Damaged gutters are often the cause of a leaky roof because the water will run directly down the exterior of the house.

Try working on the curb appeal of your home. Try improving the outside of your home by having a nicely mowed lawn with trimmed edges. Add a few well-placed shrubs. Sweep your walkway. Get the the exterior walls and roof power washed. Make sure the windows are gleaming. All of this can make a lasting impression when someone first sees your house.

Millions of homeowners decide every day to make some type of improvement to their homes, and many of them end up with poor results. Pay attention to the tips provided to you in this article so that you?re always making the best possible home improvement decisions when it?s time to spruce up your house.

Update on Prostate Cancer and Cryotherapy | My Site Mushroom

The prostate gland is a walnut-sized gland that is attached to the bottom of the bladder deep in the male pelvis. The prostate gland wraps around the male urethra as it arises from the bladder, and several ducts that run between the prostate gland and the urethra allow the prostatic secretions to be expelled into the urethra at the time of ejaculation. These prostatic secretions, which constitute about 20 percent of the volume of semen, help to create the optimal chemical environment for sperm to thrive and migrate within the female genital tract, thereby enhancing sperm function.

Prostate cancer is the most common non-skin cancer that occurs in men, and the second most common cause of cancer death in men. In 2009, an estimated 192,000 new cases of prostate cancer will be diagnosed, and approximately 27,000 men will die of this disease. Prostate cancer afflicts 1 out of every 6 American men during their lifetimes, and accounts for 25 percent of all cancer diagnoses in men (similar to the percentage of breast cancer cases among all cancer cases diagnosed in women).

As with the great majority of breast cancer cases, most prostate cancers appear to be stimulated to grow and spread by sex hormones produced by the gonads (and, specifically, by testosterone and other androgens produced by the testes, and by other tissues in the body).

Almost exactly a year ago, I wrote about the emergence of cryoablation as a treatment for prostate cancer (Cryoablation and Prostate Cancer). Cryoablation uses slender probes to freeze tumors and surrounding normal tissue, and has been proposed as an alternative to surgery or radiation therapy for the treatment of prostate cancer. In July of 2008, I noted that, in most respects, cryoablation appeared to compare favorably with surgery and radiation therapy, although the incidence of impotence appeared to be much higher with cryoablation than with standard prostate cancer treatments. I also noted, at the time, that there was no prospective, randomized clinical research data available with which to make a credible and direct comparison between cryoablation and other more established treatments for prostate cancer. Now, a new prospective, randomized clinical research trial comparing cryoablation with radiation therapy has reported its early results in the journal Cancer.

In this Canadian study, 244 men with newly diagnosed prostate cancer localized to the prostate gland were randomly assigned to undergo either standard external beam radiation treatment or cyroablation. These volunteers with prostate cancer were then followed for 3 years after completing their treatment. Because it is still too early to draw conclusions about cancer recurrence and survival in this group of prostate cancer patients, this preliminary report addresses quality-of-life issues related to these two forms of cancer therapy.

The men who underwent cryoablation reported more difficulties with urination than the men who were treated with radiation early after treatment, although these symptoms resolved over time. The cryotherapy group also reported significantly higher rates of long-term impotence when compared with the men who received radiation therapy. In fact, 3 years after treatment, there was a 13 percent greater incidence of moderate-to-severe sexual dysfunction among the cryotherapy group of men when compared to the radiation therapy group.

The preliminary results of this prospective, randomized clinical research trial suggest that the main long-term quality-of-life difference between cryotherapy and radiation therapy is a significantly greater incidence of long-term sexual dysfunction following cryotherapy.

As I concluded in my last update of cryotherapy for prostate cancer treatment, one year ago, the long-term benefits and risks of this form of treatment are not fully understood at this time, as we lack mature long-term prospective, randomized clinical research data with which to make prudent decisions about this treatment modality. Aside from a higher incidence of sexual dysfunction following cryotherapy, it is still too soon to tell if the long-term survival outcomes with cryotherapy will compare favorably with radiation therapy and surgery. Therefore, for now, I can only recommend cryotherapy for prostate cancer if it is performed within an approved clinical research trial. Stay tuned for further updates on this topic as they become available.

There are two generations of iPad on shelves, and AT&T has confirmed that its "record numbers" from Friday include both of them. More »
